About the role

Your Community Health is seeking an enthusiastic individual to contribute to our Social Support and Men's Shed Programs. Assist in the day-to-day implementation of the programs under direction of the Program's Coordinator and Manager. Support carers by providing respite, assistance, advice and understanding on issues relation to the caring role.

The role is part time (5 hours per day) for a total of 25 hours per week. Monday - Friday.  

As a special measure this position is designated for a person with lived experience as an LGBTIQA+ individual. Please identify this aspect of the role within your cover letter.  

Duties:

  • Partner with consumers to encourage consumer directed decision making to plan, implement and evaluate group activities and gentle exercises.
  • Work collaboratively with the Social Support Worker team to provide activities that enhance participant's wellbeing, encourages independence and reablement and meets their diverse needs.
  • Value and embrace the diverse needs of individuals and ensure the program reflects diversity and inclusive practices.
  • Support quality consumer documentation requirements.

Skills & experience

  • Enjoyment working with older people and people with disabilities living in the community, including multiple sclerosis and dementia.
  • Understanding and appreciation of the needs of participants who are aged and/or have a disability.
  • Sensitivity to issues relating to ageing, physical and intellectual disabilities, mental illness and diverse cultural needs.
  • Commitment to safety and quality assurance for participants and staff.
  • Verbal and written communications skills when working with people in community setting.
