As the Assessment and Facilitation Officer you will complete holistic assessments of need to support the development of goal directed support plans promoting a strengths-based approach in partnership with service providers to ensure that individual client needs can be met.
Home based Assessments for the HACC PYP must be completed within Department of Families, Fairness and Housing (DFFH) and Home and Community Care Program for Younger People (HACC PYP) guidelines. This includes supporting younger people with disabilities, or chronic health conditions to access government funded services or link into other government programs such as the NDIS appropriate to their needs.
Home and Community Care Program for Younger People (HACC-PYP) is for people aged under 65 and Aboriginal people aged under 50 who need assistance with activities of daily living, including personal care, dressing, house cleaning, preparing meals, property maintenance, and community access. The program has been designed to support people with disabilities, chronic health condition or mental health challenges and their respective carers to live as independently as possible in their own homes.
