ABOUT ROLE

  • To provide support, case management to clients who are homeless or at risk of homelessness presenting at the Salvation Army Barwon Homelessness Services.
  • Undertake intensive client assessment for intake and the provision of information and referral when needed in line with the Child Safe Code of Conduct
  • Provide and offer clients suitable housing that will enhance their ability to work towards their goals
  • Participate in and conduct an intensive induction for all clients entering into accommodation
  • Develop appropriate holistic case plans for all clients with a strength based, solution focused and recovery oriented approach with specific focus on housing options
  • Conduct exit plan interviews with all clients exiting the program
  • Present clients with a feedback form to complete
  • Provide an environment and activities that enhance clients ability and wellbeing to enable them to work towards their goals in a supportive and structured manner
  • Assist clients to access appropriate day activities, training and development programs
  • Link clients into job networks and other vocation programs

ABOUT YOU

  • Tertiary qualification in welfare, community development or related field with significant experience is preferred.
  • A national police record check is required
  • A current and valid Working with Children Check or equivalent
  • A current Drivers licence
