The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

The Salvation Army Housing / Transitional Support Program(s) operate throughout Victoria East including The Salvation Army Homelessness East, The Salvation Army Homelessness Gippsland, and The Salvation Army Homelessness Peninsula.

Our service provides case management, outreach and support to people who are homeless or at risk of homelessness to access safe, affordable accommodation and prevent their return to the homelessness service system. Homelessness transition support can be provided, irrespective of a person's existing living arrangement for example in rooming houses, transitional housing, squats, rough sleeping, and informal housing arrangements.

We are currently seeking a values-driven and proactive Case Manager to provide information, advice, advocacy, liaison and referrals to people who are assisted using a strength-based approach to identify goals to empower them to work towards establishing safe, secure, affordable housing. Additionally, case managers develop person centred and holistic case (and or safety) plans to ensure the individual needs and risks of each client is met throughout their service period with The Salvation Army Homelessness Victoria East.

You will successfully:

  • Develop strong networking relationships are formed and maintained with relevant stakeholders to ensure coordination and responsiveness to meet the needs of homeless people
  • Ensure effective relationships are established with clients that encourage self-determination and participation
  • Provide ongoing assessment of the client's issues, needs and circumstances from initial intake to exit
  • Develop individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients

This is a permanent full-time position, reporting to the Team Leader based in Leongatha.

You will have:

  • Relevant tertiary qualification in social work, community services, youth work, psychology or community development - diploma level (minimum)
  • Relevant experience in a social service environment
  • Knowledge of theories of human development and how they impact on current day approaches and interventions in the social services sector
  • Ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness
  • Understands and applies homelessness models, approaches, theories and practices when dealing with clients
  • Expertise in working with clients with multiple vulnerabilities, histories of trauma, and complex needs
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
