The local team is dedicated to delivering personalised, quality home care services in the Sunshine Coast. They are accredited by the Commonwealth Government as an Approved Provider of Home Care Packages specialising in offering home care services to support the needs of older Australians.
The Position
Providing personal and community access care to customers, your responsibilities will include but are not limited to:
- Planning and oversight of the scheduling of support workers and service providers for clients
- Responsible for the development and review of Care Plans in consultation with customers and their families
- Ensure individual consumer budgets are accurately recorded and monitored regularly
- Regularly reviewing customer's Care Plans; requests for upgrades and supplements; leave and other related activities
- Accurate maintaining and updating of Care Management systems and communication tools
The Successful Applicant
To be considered for this opportunity you must meet the following criteria:
- Tertiary Management Diploma, Certificate III or IV in Individual Support, Home & Community Services, Aged Care, Disability or Nursing Degree highly desirable
- At least 12 months experience in a similar role
- An Open Australian Drivers Licence and car insurance
- High level of customer service
- Hold or have the ability to hold or apply for a Criminal History Check
Candidates must have the covid 19 vaccination and be willing to use their vehicle for work purposes
