Omnia Community Health was established through the merger in January 2026 of Sunbury and Cobaw Community Health and Nexus Primary Health.  Bringing together the deep community connections, shared values and specialist expertise of both organisations, Omnia builds on more than 50 years of experience delivering health and wellbeing services within the not-for-profit sector.

We offer a broad range of services across the lifespan including general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing.  These services are delivered from our sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach across surrounding communities.

We are committed to enhancing access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.

About the Role

The Family Safety Counsellor – Women’s Contact (Partner Safety Contact Worker) provides counselling and support to current or former partners of participants in our Men’s Case Management and Mens Behaviour Change Programs The role involves completing risk assessments, developing safety plans, providing referrals, and supporting partner wellbeing throughout the program duration.
