On 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health merged to form a new, unified not‑for‑profit community health organisation, Omnia Community Health. This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high‑quality and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas.
The new organisation brings together the deep community connections, shared values and specialist expertise of both entities. Services span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing. These services are delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding communities.
The Falcon Service Navigator works with adolescent males experiencing or at risk of using family, domestic and sexual violence in Sunbury. Using a community engagement and development approach, the Service Navigator will identify local opportunities to engage adolescent males in community supports beyond individual therapy using a holistic approach.
The successful applicant will hold a relevant qualification in social work, welfare, counselling, or a comparable discipline, and demonstrate experience working within the field.
Working alongside the Family Services Hume team, you will be part of a dedicated group of professionals supporting vulnerable families across the region. We are seeking someone with experience and a genuine passion for working in the family violence sector, with a particular focus on adolescent males. You will bring an understanding of, and experience in, working from a trauma-informed lens, with the ability to engage effectively with young people in Sunbury.
