About New Dawn

New Dawn Support Services Pty Ltd is an approved NDIS provider with its head office located in Cranbourne, Melbourne. We provide person centred disability support services across Australia, including Melbourne, Perth and Brisbane. At New Dawn, we do not just provide services. We build bridges that help people live with dignity, confidence and independence.

Our services include community participation, innovative community participation, life skills development, personal care support, daily living assistance, support coordination, life stage transition support, household tasks and behaviour support. We are committed to supporting NDIS participants to engage with their local community, develop practical life skills and make choices that improve their quality of life.

We currently have three Community Worker positions available in Morayfield, Brisbane. We are looking for dedicated, caring and reliable Community Workers who share our values of Care, Respect, Courage and Independence, and who are passionate about helping NDIS participants thrive in their everyday life and community.

The Role

As a Community Worker with us, you aren’t just a "case manager"—you are a mentor, an advocate, and a community builder. Your goal is to help individuals build the confidence they need to lead independent lives.

On a typical day, you will:

  • Meet with clients and their families to deeply understand their goals and design personalized support plans that actually work.
  • Create and maintain meaningful community connections, ensuring our clients aren't just "present" in the community, but truly included.
  • Assess community needs and available local resources, including health, welfare, housing, employment, training and social participation services, to identify practical support options for NDIS participants.
  • Liaise with community organisations, welfare agencies, government bodies, service providers and local businesses to promote access to community resources and improve participant inclusion.
  • Monitor participant progress against agreed support goals, evaluate the effectiveness of community participation activities, and update support strategies where required in consultation with clients, families and relevant stakeholders.
  • Lead workshops, social outings, and skillbuilding activities that foster independence.
  • Be the expert guide, linking clients to essential services in health, housing, and employment.

  • Maintain high-quality case notes and reports to ensure we are always delivering the best possible care.
  • Meet with families to assess their unique needs and turn "someday" goals into a personalized, actionable plan for independence.
  • Navigate the NDIS maze to link clients with the health, housing, and social services they need to thrive in their community.
  • Provide hands-on guidance through life’s practical and emotional hurdles, helping clients build the confidence to lead their own lives.

About You

You are someone who looks past the "file" and sees the person. You’re organized enough to handle the paperwork but empathetic enough to handle a crisis with a calm, steady hand.

To be successful, you’ll need:

  • Diploma or higher in Community Services, Social Work, Welfare, or related field (Cert IV considered with strong experience)
  • Minimum 2 years’ experience in NDIS, community, social, or disability services
  • Proven experience in community development, welfare support, or related services
  • Ability to assess client and community needs and design culturally appropriate programs
  • Experience supporting clients with mental health challenges, trauma, or disadvantage
  • Strong communication skills to engage with clients, families, and community partners
  • Knowledge of health, welfare, housing, employment, and training resources
  • Cultural sensitivity, patience, resilience, and excellent time management
  • Ability to manage incidents and crisis situations calmly
  • Practical skills in household support (meal preparation, basic housekeeping, group activities)
  • Flexibility to work evenings, weekends, and sleepovers when needed
  • Reliable car and full licence to support client transport (kilometres and tolls reimbursed)
  • Mandatory checks: Police Check, Working with Children/Vulnerable Persons Check, NDIS Worker Screening
