The Opportunity

Working within Council's Young, Inclusive and Ageing Communities, you will be responsible for the coordination of flexible and tailored support services in accordance with clients' care plans and Commonwealth and State program guidelines. This role will support Moonee Valley as an 'age-friendly city' through initiatives that promote community inclusion and acceptance, healthy and active living, restorative care and a broad range of appropriate support services.

This role reports directly to the Coordinator Community Care.

Key roles and responsibilities include:

  • Provide holistic assessment, referral and care planning to eligible residents, including goal directed care planning and ongoing review with a focus on consumer directed care.
  • Develop individual support plans in consultation with service recipients, their families, carers, and nominated advocates.
  • Liaise with service delivery providers to establish service partnerships and monitor and advocate for client's care needs.
  • Assist with contract management of the service delivery partners, including but not exclusive to; monitoring providers compliance, performance and client satisfaction.
  • Undertake assessments and review of recipients' service needs and focus on consumer directed care.
  • Maintain accurate, up to date and secure case notes, assessment, referral and care planning records and client files including documentation within Aged and Disability Services' Client Management System (CMS) and relevant funding body systems.
  • Ensure administrative functions are completed in an accurate and timely manner including statistical collection, activity monitoring and reporting.
  • Comply with corporate and clinical governance processes and funded program guidelines.
  • Participate in the development of an efficient, effective and customer-focused care management team that operates as a business unit with a strong community focus
  • Participate in the emergency relief efforts to support recovery during a municipal emergency/ event

You:

  • Bachelor in Community Development, Social Work, Allied Health or Nursing with relevant industry experience.
  • Comprehensive understanding of client centred care planning and an ability to communicate with families, staff and external agencies
  • Comprehensive knowledge of Commonwealth and State funded programs and the local service system that provides support and services to older people and people with disability
  • Demonstrated proficiency and experience in providing case management to complex clients, including informed and effective decision making, particularly in situations requiring an urgent and immediate response
