About the program

Wellways Australia is the first point of contact for all Australian Government Carer Gateway funded services in South West Sydney and across Queensland, aiming to help carers access practical information and advice and connect with services in their local area. Carer supports and services are delivered using a network of local Carer Gateway regional service providers Carers and those who provide a caring role to someone in their life who has a disability, mental health issue, chronic health condition, terminal illness, or are frailly aged.

About the role

The Community Engagement Support Worker is a newly created role that is responsible for undertaking intake, assessment and planning services for Carers accessing support provided by the Carer Gateway across South East Queensland.

Key responsibilities include, however, are not limited to:

  • Identifying hidden or hard to reach carers and communities
  • Providing Intake, Planning and Assessment services to Carers
  • Providing Carers with information on internal and external programs/services
  • Developing action plans and Ensuring action plans focus on providing service solutions that maintain and strengthen Carers health and wellbeing

Skills & Experience:

  • Demonstrated knowledge of caring roles and the impact on Carers health and wellbeing to determine support requirements
  • Experience in coordinating responsive individualised support to individuals, families, young people and children with caring roles
  • Effective communication skills, verbal and written, including the ability to develop reports, public speaking and conference presentations

Pre-employment requirements:

  • Certificate IV in Community Development
  • Evidence of working rights within Australia such as a passport or birth certificate
  • National Police Records Check
  • Blue Card and Yellow Card (Current or willing to obtain prior to employment. Please note you will not be able to commence until you have received a Blue Card)
  • NDIS Worker Orientation Module Certificate: https://www.ndiscommission.gov.au/workers/support-for-workers/training-course
