HMS Community is Australia’s first Community Paramedic-led healthcare charity, delivering preventative, person-centred care across Melbourne’s Northern Suburbs and the Macedon Ranges.

By providing accessible healthcare in people’s homes, communities, and our clinic, HMS Community helps prevent unnecessary hospital presentations—saving ambulance services approximately 90 hours of callouts each week, or around $1.4 million in healthcare costs.

Our holistic, community-based model supports physical, social, and emotional wellbeing, empowering people to live healthier, more independent lives.

Care is delivered by a multidisciplinary team of Community Paramedics, Nurse Practitioners, Community Nurses, Support Partners, and allied health professionals through homecare, preventative outreach, a specialist clinic, and the Community Health Hub in Gisborne, offering services such as Chinese medicine, physiotherapy, and counselling.

ABOUT THE ROLE

We are looking to hire one experienced support worker or an enrolled nurse to support an individual in Gisborne to cover a leave period from 9/2/26-28/2/26. With paid on the job training beginning asap and potential for ongoing work after the leave period.

About the Individual:

12 years old (she/her), lives with dystonic cerebral palsy. Uses a wheelchair, is non-verbal and requires care in all areas of her life.

Tasks involved:

  • Personal care and continence care
  • Meal assistance – via peg
  • Medication assistance – via peg
  • Manual handling – ceiling hoist
  • Social support and respite for the family

Shift times required:

  • A mix of 4pm-8pm and 10am-4pm shifts are required, shared amongst a team
  • Period of daily care (including weekends) required - dates TBC
  • Ongoing shifts are Wednesday & Thursday 4pm-8pm with some additional care over the weekends, during the day and during the school holidays

ABOUT YOU

We are looking for an experienced Support Worker, Enrolled Nurse, or a 2nd-year Paramedic or Nursing student who is compassionate, reliable, and committed to providing high-quality, person-centred care.

The ideal candidate will have some experience in disability, complex care, or paediatric in-home care, but we also welcome applications from students or candidates who are eager to learn, develop their skills, and gain hands-on experience in a supportive environment.
