About the Role

The Wellbeing Coordinator is responsible for promoting the health, wellbeing and independence of Good Shepherd Sisters and women in Sydney.

This role combines direct, person-centred support with the coordination of health, aged care and community services to help women maintain their wellbeing, autonomy and quality of life. Working closely with a range of service providers and stakeholders, you will support women to age well, stay connected to their communities and access the services they need to thrive.  

Key Responsibilities

  • Promote the health, wellbeing and quality of life of Good Shepherd Sisters and women in Sydney
  • Support healthy ageing, wellness and enablement through evidence-based and person-centred approaches
  • Identify health and wellbeing concerns and implement responsive support strategies
  • Coordinate aged care, health and community services to support optimal outcomes
  • Support women to make informed decisions about their care and future planning
  • Advocate on behalf of individuals when engaging with healthcare providers and external services
  • Coordinate medical appointments, transport and hospital visits as required
  • Support participation in community, mission and social activities
  • Coordinate milestone events including jubilees, birthdays and other significant occasions
  • Maintain accurate case notes, documentation and reporting  

About you

  • Registered Nurse
  • Demonstrated ability to navigate aged care systems and external support services
  • Strong commitment to person-centred care and healthy ageing principles
  • Experience coordinating services and working collaboratively with a range of stakeholders
  • Excellent communication, relationship-building and organisational skills
  • Current First Aid Certificate Current driver's license
  • Experience in aged or community care setting
  • Ability to work independently while contributing to a collaborative team environment
  • Commitment to social justice and alignment with the mission and values of GSANZ
