About the role

The Community Connector occupies a pivotal and dynamic position at FWHC, responsible for coordinating and ensuring holistic, integrated and seamless client pathways for women delivered from both the centre and on an outreach basis. Based on a social model of health, the role operates within an interdisciplinary team at FWHC aimed at collectively improving the wellbeing outcomes for women. The Community Connector is also key in establishing and maintaining networks and developing activities that progress systemic advocacy and community awareness.

Duties of the position: 

Service Provision

  1. Provide entry point intake and assessment for women referred to FWHC in partnership with members of a interdisciplinary team.
  2. Develop person centred, strengths based case management plans in collaboration with clients that reflect the context of their physical, social and cultural environments, their experiences, and their preferences.
  3. Provide information to and warm referrals for clients, as required.
  4. Monitor, adapt and review case management plans with clients and other service providers as needed and appropriate.
  5. Maintain contact with women on the waitlist for appointments in partnership with members of a multidisciplinary team.
  6. Provide information and psychosocial support to significant others of women referred, as appropriate.
  7. Prepare and maintain client notes and ensure that client data remains confidential and is securely stored and managed.
  8. Consult with other health professionals and community services as required, and provide reports to referring agencies as needed and appropriate in partnership with members of a multidisciplinary team.
  9. Design, deliver and evaluate education, information and psychosocial groups based on client needs.
  10. Assist in the establishment and improvement of client pathways and centre policies and procedures that reflect the philosophy of the FWHC.
  11. Remain abreast of current trends in the delivery of health services to women.
  12. Engage with women to co-design changes to existing or new services.
  13. Maintain a high level of professional standards at all times.
  14. Other duties as required.

Community development

  1. Strengthen existing and develop new partnerships and networks with key stakeholders aimed at improving outcomes for women.
  2. Develop strategies and maintain community engagement
  3. Represent the FWHC, and the needs of women using the service, on external forums and at other events as directed.
  4. Advise and work with the CEO to identify and implement policy, education and research opportunities aimed at addressing issues that impact women, and improving social, economic and cultural outcomes.   

Data Collection

  1. Collect statistical data on clients and enter into the database on a regular basis and within designated timelines as directed.
  2. Provide case studies and any other information relevant for reporting purposes. 

Professional development

  1. Participate in supervision on a monthly basis with a designated supervisor.
  2. Continue with ongoing professional development relevant to this position.

Teamwork

  1. Contribute to the maintenance and growth of FWHC as a centre of excellence in women’s services with programs that are based on community need.
  2. Contribute to ensuring that new areas and opportunities for FWHC services are identified and developed where there are emerging needs.
  3. Undertake shared team tasks as required, as a member of a multidisciplinary team.
  4. Attend monthly staff meetings and regularly attend other FWHC meetings as required.
