Forensicare is the statutory agency responsible for the provision of adult forensic mental health services across Victoria. Forensicare is situated across five sites across Victoria with its largest site being Thomas Embling Hospital, located in Fairfield just 15 minutes north of the Melbourne CBD. Thomas Embling Hospital is a 116 bed secure hospital providing acute care, rehabilitation and continuing care services.

About the role:

The role of the Social Worker is to plan, implement and evaluate a high standard of social work service delivery, which privileges the lived experience of consumers and their families and carers. The Social Worker is an advocate for consumers and carers, assisting them to navigate the multi-layered and complex service systems of which they are a part of, applying social work approaches and contributing a unique perspective to the multi-disciplinary team.

Duties:

• Display initiative in social work service delivery.
• Contribute to person-centred care and treatment, risk assessments and discharge planning.
• Liaison with external stakeholders, including accommodation placements and NDIS support planning.
• Promote and develop Aboriginal Service Officer and Cultural Portfolio. 

Qualification, skills and experience:

• Approved degree in Social Work and eligibility to join AASW.
• Demonstrated commitment to mental health consumer recovery.
• Experience in the mental health field.
• Demonstrated commitment to advocacy, brokerage and social justice.
