About the role:
We are seeking passionate, caring and experienced Support Coordinators who have an excellent understanding of the NDIS. Your work will support people to achieve their individual lifestyle goals through building capabilities to best meet their needs, goals and desires within their budget. Essential Skills & Experience:
- At least 1 year experience as a NDIS LAC/Planner or Support Coordinator in a similar role within the healthcare industry
- Experience coordinating specialist services for those with complex needs working with cross-functional teams
- Strong commitment to principles of enhancing independence and client empowerment
- A positive attitude with a flexible and adaptive nature to respond to the changing needs of our clients
- Excellent project management, communication, and interpersonal skills
- Happy to travel up to an hour to see clients.
Desirable selection criteria:
- Relevant qualification and/or extensive experience in allied health, disability, or community services
- A strong understanding of the disability sector.
Responsibilities and Duties:
- Establishing relationships with relevant support providers
- Managing and maintaining participant plans
- Working with participants to achieve their goals in line with their NDIS plans
- Use a coaching approach to assist participants in capacity building
- Encourage and facilitate participant inclusion in community activities
- Working in a respectful way with people with varying support needs, their families, support networks and community.
- Maintain accurate records of participant planning activities
- Attend and facilitate all participant support meetings
- Supporting evidence-based practice through your high level of administrative skills, documenting outcomes for individual NDIS participants, evaluating service delivery and identifying challenges and opportunities in the new environment.
- Using your outstanding communication skills to create documents and reports, construct proposals and funding submissions and record and analyse data relevant to service delivery.
To be considered for this role, you must provide the following:
- Proven experience working as a Support Coordinator supporting people with disabilities or within the aged care industry
- NDIS Worker Screening Clearance
- Current Driver’s License & your own car
- Knowledge of the NDIS
- Satisfactory National Police Check (No more than three years old)
- Satisfactory Working with Children's Check (Employee).
