Divine Support Care Pty Ltd is an approved NDIS provider delivering person-centred disability and community services across Brisbane and surrounding areas.

Our registered supports include community participation, life-skills development, supported independent living, assistance with personal activities, transport, household tasks, accommodation and tenancy support, community nursing and group-based activities.

We are looking for a compassionate, reliable and experienced Community Worker to join our team in a full-time permanent position.

About the role

As a Community Worker, you will work closely with participants, their families, community organisations and other service providers to identify individual and community needs, coordinate appropriate supports and help participants achieve greater independence, inclusion and participation in the community.

This is more than a personal-care role. The position requires sound judgment, communication, planning, documentation and service-coordination skills.

Key responsibilities

Your responsibilities will include:

• Assessing participants’ needs, strengths, goals and available community resources
• Developing, implementing and reviewing individual support, education and community-participation programs
• Interviewing participants and identifying the nature and extent of the difficulties or barriers they experience
• Supporting participants to develop practical life skills, independence and confidence
• Facilitating access to education, employment, housing, health, recreation and welfare services
• Supporting participants to engage in social, recreational and community activities
• Monitoring participants’ progress and maintaining accurate case notes, reports and service records
• Referring participants to government agencies, allied health professionals, welfare organisations and other appropriate services
• Liaising with families, support coordinators, community organisations, government bodies and other service providers
• Promoting awareness of available community resources and services
• Advocating for participants and supporting them to exercise choice, control and independence
• Assisting with transition planning, accommodation or tenancy matters and access to community-based supports
• Identifying and responding appropriately to risks, incidents, safeguarding concerns and changes in participants’ circumstances
• Contributing to service reviews, team meetings and continuous improvement activities
• Ensuring all services are delivered respectfully and in accordance with participant support plans, organisational policies and the NDIS Code of Conduct

Qualifications and experience

To be considered for this position, you must have:

• A minimum Diploma of Community Services, Community Development, Welfare, Disability, Social Work or another closely related field
• Relevant experience in community services, disability services, welfare, community development or a closely related area
• Experience assessing participants’ needs and developing or implementing support programs
• Strong knowledge of person-centred practice, participant choice and control, dignity, privacy and safeguarding
• Good written communication skills and the ability to prepare professional case notes, reports and incident records
• Strong interpersonal skills and the ability to communicate respectfully with participants from diverse cultural and social backgrounds
• The ability to work independently and as part of a multidisciplinary team
• Good organisational, time-management and problem-solving skills

Mandatory clearances and licences

The successful applicant must hold, or be willing and eligible to obtain before commencing:

• A current NDIS Worker Screening Clearance
• A current Queensland Blue Card or Working with Children Check where the role involves working with children
• A current National Police Check
• Current First Aid and CPR certificates
• A current unrestricted Australian driver’s licence
• Access to a reliable, registered and appropriately insured vehicle
• Completion of the NDIS Worker Orientation Module
• Evidence of Australian working rights

Workers directly providing NDIS supports for a registered provider will generally require an NDIS Worker Screening Clearance where the position is a risk-assessed role. In Queensland, a Blue Card is also required where the worker provides disability services to children.
