Communify is seeking an experienced Community Development and Transition Coordinator to play a pivotal role in establishing and transitioning the Redcliffe Medicare Mental Health Centre (MMHC) into a thriving, community-connected service.

This is an exciting opportunity for someone who is passionate about community engagement, stakeholder collaboration and improving access to mental health services. Working alongside the Centre Manager and a multidisciplinary team, you'll help ensure the Centre is designed with and for the community through meaningful consultation, co-design and partnership development.

About the Role

Reporting to the Centre Manager, you will coordinate community development and transition activities that support the successful implementation and ongoing development of the Medicare Mental Health Centre.

You'll work closely with community members, people with lived experience, health providers, government agencies and partner organisations to ensure the service remains responsive, culturally safe and informed by community needs.

Key responsibilities include:

  • Developing and delivering community engagement, consultation and co-design activities
  • Building strong partnerships across health, government and community sectors
  • Supporting the transition of the Redcliffe MMHC into a full Medicare Mental Health Centre
  • Coordinating stakeholder forums, meetings and community events
  • Supporting the Mental Health Care Partners Pilot Program
  • Contributing to research, evaluation and continuous improvement initiatives
  • Preparing project reports and maintaining accurate records
  • Promoting awareness of the Centre throughout the Moreton Bay community

About You

You are an experienced community development or project professional who enjoys bringing people together to create meaningful change.

You'll bring:

  • Relevant tertiary qualifications in Community Development, Public Health, Health Promotion, Social Sciences or a related discipline
  • Demonstrated experience in community development, stakeholder engagement and partnership building
  • Experience delivering consultation and co-design projects
  • Strong project coordination and organisational skills
  • Excellent communication and facilitation abilities
  • Experience undertaking research, evaluation and reporting
  • A collaborative approach and commitment to trauma-informed, recovery-oriented and culturally safe practice

Experience working within mental health or alcohol and other drug services will be highly regarded.
