At Better Health Network (BHN), we're passionate about supporting people to live healthier, more connected lives. We’re looking for two, part-time contract Support Workers in our Better Homes Better Health program, to help public housing residents navigate one of life's biggest changes—relocating to a new home—while ensuring they remain connected to the services, supports and communities they need to thrive.

About the role

This is a community-based role where you'll work directly with public housing residents who are relocating into social housing. You'll build trusted relationships, provide practical support, connect people with services, and help strengthen community participation and wellbeing.

No two days will be the same. You might be:

  • Supporting residents through the relocation process, including home viewings, packing, settling in and connecting with local supports.
  • Building relationships through outreach and regular community engagement.
  • Facilitating community activities, groups and health promotion events.
  • Helping people access health, housing and social support services.
  • Providing transport and practical assistance to appointments and community activities.
  • Working alongside a multidisciplinary team to support sustainable tenancies and improved wellbeing outcomes. 

About you

You're a warm, approachable and proactive community services professional who enjoys building genuine relationships and supporting people to achieve their goals.

You'll bring:

  • A Certificate IV or Diploma in Community Services, Health or a related field (or equivalent experience).
  • At least 3 years' experience in community, health or social services.
  • Experience in outreach, community engagement and/or service navigation.
  • Strong communication skills and the ability to work with people from diverse backgrounds and with complex needs.
  • A commitment to person-centred, culturally responsive practice and the Social Model of Health.
  • A current driver's licence.

Nice to have

  • Experience or lived understanding of public housing communities, CALD communities or LGBTI+ communities.
  • A language other than English.
  • Knowledge of services and community resources across Melbourne's south-east.

Location

Based at Prahran, with travel across community locations as required.
