We are seeking a qualified and experienced Therapeutic Case Manager to join our friendly and supportive Enhanced Care Opportunities (ECO) team at our Dandenong office. We are looking for someone with strong leadership and supervision experience who is able to effectively engage with and work with disadvantaged young people, their caregivers and family members. The Therapeutic Case Manager will oversee the care team of specialist professionals in delivering high level of care for complex foster care young people in ECO. This part time role has a focus on coordinating the care team involved with each of our young people to ensure enhanced levels of care.

The Position: 

The Therapeutic Case Manager will support young people and their carers and family members. The position objectives are:

  • The role of the Therapeutic Case Manager is to provide leadership and coordination of the care team for each child placed in the program.
  • This role is responsible for providing support and advice to clients and carers, and to lead the multi-disciplinary team to develop and implement trauma informed plans. 
  • The role is responsible for providing effective leadership and for the delivery of a professional service that is therapeutic and culturally appropriate.

To Be Successful in this Position You Will Need:

  • Tertiary qualifications, preferably in social work / psychology, youth work or equivalent.
  • Provide leadership to each care team to improve outcomes for young people placed in ECO foster care placements.
  • Commitment to the principles and practice of quality care and coordinate the team of professionals involved to achieve positive outcomes.
  • To have experience in leading and supervising multi-disciplinary teams.

Please Note: It is mandatory for the successful applicant to hold a current Working with Children Check and be prepared to undergo a National Criminal History Check prior to commencement.
