We are a small NDIS registered Support Coordination company, with a client-centred practice that knows the participant is the expert in their life. Our size gives us the flexibility to adapt our service with the participant to implement their plan to maximise benefits to them.
We have built an excellent reputation with our participants, referrers, and the providers we liaise with for exemplary service delivery.
The successful applicant must be comfortable with frequent travel.
The position will initially be orientation based in Caulfield North, and client visits are in the western suburbs of Melbourne.
As the successful applicant, you will have the background in, knowledge and understanding of the NDIS, at least two years' experience working as a support coordinator, planner, or LAC for the NDIS.
You demonstrate your interest in working with people experiencing complex and severe psychosocial disability and the related challenges that arise, with a clear commitment to making difference.
This should be underpinned with attention to detail, effective time management and reporting skills to positively affect the outcome for clients.
Critical to the role is the ability to work independently, with a solution focused approach. This includes working with stakeholders at all levels to drive a positive outcome for your client!
You will have a qualification at diploma level or higher in Mental Health or including Mental Health, or equivalent in experience.
You will own an insured car that would be appropriate for transporting clients when required, reimbursed at 85c per km.
