We are looking for an Intake Coordinator to receive referrals from health facilities in the Murrumbidgee region, and to assess client referrals for eligibility to the NSW Health Out of Hospital Care Program (this includes ComPacks, SASH, End of Life packages). OHC packages are non-clinical packages of care.

This is a new permanent role reporting to Out of Hospital Care Program Manager, and based within our Chatswood office with the opportunity for flexible work arrangements including the potential for part-time or full-time hours, and working from home.

About You:

• Previous work experience in intake/assessment/referral
• Customer service orientation
• Good relationship building skills with local health district health professionals, and administrators
• Exceptional administrative skills and attention to detail
• Comfortable and proficient using an online client management system
• Experience working in public health environment, preferred
• ComPacks, SASH and End of Life packages experience, advantageous
