About the Role

The Integration Officer Care Finder position supports the strategic objectives of Western NSW Primary Health Network (WNSWPHN) to lead, support and strengthen person centred primary health care, by developing and coordinating activities related to older people's health, aged care and palliative care.

The role will focus on the management, needs assessment, commissioning, contract management, implementation, and coordination of WNSW PHN care finder program. The program will strengthen support for care finders, including access to local and regional communities of practice to build their knowledge and skills. The goal of this role is to embed care finders as an integrated part of an integrated local aged care system. This will also involve collecting data and information to support both continuous quality improvement (CQI) initiatives and evaluation of the care finder program.

The position will provide contract management and project support to manage and maintain a network of care finders across the WNSW PHN region. Care finders provide assistance to people within the care finder target population to understand and access aged care and connect with relevant supports in the community. This role will undertake other aged care projects and tasks as determined by organisational need.
