About the Role

The Integration Officer Aged & Palliative Care - Allied Health position in the Aged & Palliative Care team, supports the strategic objectives of Western NSW Primary Health Network (WNSW PHN) to lead support and strengthen person centred primary health care, by developing and coordinating activities to related to older people's health, aged care and palliative care.

This role will primarily support and strengthen allied health roles in primary health. The role will have a dual focus on 1.implementation of The Aged Care On-site Pharmacist Measure (ACOP) in residential aged care homes (RACH), and 2. the coordination of the engagement and support for Allied Health sector in Western NSW. This position will be responsible for the coordination with internal and external stakeholders to ensure the successful planning and delivery of allied health projects for the Aged and Palliative Care team.
