About the Navigator Program

The Outer East Navigator Program is an exciting partnership between Uniting Victoria and Tasmania Ltd (Uniting), Victorian Aboriginal Child Care Agency (VACCA) and the Australian Childhood Foundation (ACF).

The program will utilise a trauma informed, culturally safe assertive case management model to respond to challenges experienced by young people aged 12 – 17 years with the goal of re-engaging disengaged learners in education.

The Navigator Program utilises a Capability approach that understands how multiple characteristics can interact to compound educational disengagement that can result in disconnection and a lack of a sense of purpose and self-worth for young people.  This amplifies service barriers and exacerbates social and economic disadvantage, and social isolation.

About the opportunity

The partnership is looking for a Case Manager, to be employed by Uniting. The Case Manager will provide direct support and service to a caseload of disengaged learners. Other duties include:

  • Liaison and collaboration with partner agencies and funding bodies
  • Working collaboratively with children and families to establish an education re-engagement plan
  • Establishing and leading a care team
  • Undertake assessments of disengaged learners
  • Maintaining client records
  • Community education

About you

To be successful in your application, you will meet the person specification in the position description, including having:

  • Demonstrable experience working with vulnerable youth and families
  • Knowledge of family services, child protection and the department of education and training, including relevant legislation and regulations
  • Understanding of the barriers experienced by disengaged learners
  • Excellent communication and organisational skills
  • Proven ability to develop strong relationships with key stakeholders and develop and implement a new program initiative
  • Knowledge and understanding of trauma informed and culturally competent practice, including knowledge and application of therapeutic parenting practices
