The Carer Intake and Review Coordinator plays a vital role in supporting carers within the Uniting Vic.Tas Carer Gateway catchment areas. This position is responsible for coordinating and scheduling Carer Star appointments, ensuring carers are connected to appropriate supports in a timely and efficient manner.
The role includes conducting initial carer intake assessments and completing review conversations to monitor wellbeing and support needs. The Coordinator also provides administrative support to the broader team as required.
Where complex or high-risk presentations are identified, the Coordinator will appropriately escalate matters to the Senior Carer Wellbeing Support Planner to ensure carers receive specialised and responsive support.
This position requires strong organisational skills, attention to detail, sound communication abilities, and a compassionate, carer-centred approach.

You will bring with you:
Desirable: Tertiary qualifications in Disability, Social Work, Health, Welfare, Community Services, Community Development, or a related discipline (or equivalent experience).
Demonstrated understanding of the role, needs, and challenges of carers, with a strong commitment to upholding their rights, wellbeing, and entitlements.
