The Wesley Care finder case worker plays a vital role in supporting vulnerable older people who require intensive assistance to navigate and access aged care and related supports.
We provide support to connect participants to relevant programs and services in the community, targeting individuals who face barriers such as:
The Wesley Care finder supports vulnerable older people who are unable to navigate the aged care system on their own and who do not have someone else to assist them. This is a frontline, community‑based role focused on ensuring older people do not miss out on the care and supports they need.
Working within a person‑centred, outreach‑based model, Care finders provide free, intensive, face‑to‑face support to help clients understand their aged care options, connect with My Aged Care, arrange assessments, choose appropriate providers, and link with community and support services. The role involves building trusted relationships and supporting people experiencing complex barriers such as social isolation, health concerns, cognitive decline, financial hardship, or cultural and language challenges.
Regular travel within office hours is a significant part of this role, with the Case Worker working across the local PHN region to meet clients in their homes and community settings.

Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 5, with capacity for salary packaging opportunities and the provision of a fully maintained motor vehicle available for private use.
