About the role

The position joins an existing Aboriginal Health Project Officer who currently leads the Integrated Team Care and Ask The Question Project activities addressing the under- identification of Aboriginal and Torres Strait Islander people in mainstream health services.

The key purpose of the role is to work with PHN colleagues to build capacity of mainstream primary care agencies to deliver culturally competent services to Aboriginal and Torres Strait Islander people, and to foster collaboration between mainstream primary care and Aboriginal and Torres Strait Islander health sectors.   

Duties:

  • Identify and address barriers faced by Aboriginal and Torres Strait Islander people when accessing mainstream primary care, including pharmacy, allied health and specialists
  • Promote mainstream primary care providers to Aboriginal and Torres Strait Islander people as a valid, trustworthy and accessible first point of health care.
  • Engage with PHN practice facilitators to help build capacity of mainstream primary care providers to manage specific Aboriginal and Torres Strait Islander health needs
  • Coordinating and contributing to education events with the PHN Workforce Development Team
  • Identifying key stakeholders, building and nurturing strong relationships

Skills & Experience:

  • Sound knowledge and understanding of issues and challenges for improving the health status and outcomes for Aboriginal and Torres Strait Islander people
  • Ability to develop rapport with Aboriginal and Torres Strait Islander patients and communities and provide culturally appropriate support 
  • Demonstrated knowledge and experience in health projects and programs
  • Work autonomously with minimal supervision and instruction
  • Degree or diploma level relevant qualification in health or social services, or Certificate IV Aboriginal and Torres Strait Primary Health Care (Community Care) or equivalent
