Western Victoria Primary Health Network (PHN) is responsible for increasing the efficiency, effectiveness and coordination of health services for patients, particularly those at risk of poor health outcomes. Western Victoria PHN has four regional centres in Ballarat, Geelong Horsham and Warrnambool.
Together with our partners and communities, Western Victoria PHN is committed to strengthening the primary healthcare system, based on local needs. Our regional centres drive local engagement and ensure strong rural and regional connections through the development of local community and clinical councils.
The position joins an existing Aboriginal Health Project Officer who currently leads the Integrated Team Care and Ask The Question Project activities addressing the under- identification of Aboriginal and Torres Strait Islander people in mainstream health services.
The key purpose of the role is to work with PHN colleagues to build capacity of mainstream primary care agencies to deliver culturally competent services to Aboriginal and Torres Strait Islander people, and to foster collaboration between mainstream primary care and Aboriginal and Torres Strait Islander health sectors.
