Homelessness and Community Services (HCS) of Uniting provide outreach and crisis support and case management to people who are homeless or at risk of homelessness, including those with tenancy issues. The services support people with the aim of breaking the cycle of homelessness through person centered support and coordination, referral, advocacy and development.

Role Overview:

This Support Worker is responsible for the provision of safe and appropriate accommodation and support services to residents through the delivery of high-quality care to service users.

Duties and Responsibilities include but are not limited to:

• Assist with intake and assessment where relevant to the role
• Develop and monitor support plans/individual case plans, based on Services Practice Guidelines
• Provide individual assistance for clients in line with support plans and identified goals

Essential Criteria:

• Certificate IV in Community Services, Mental Health, Youth Work or other relevant field
• First Aid Certificate
• 2 or more years’ experience in your field of expertise. You will be continuing to develop in your area of expertise and be expected to provide innovative ideas to solve problems in your discipline.
• Experience working with vulnerable people, relevant to the service (Men, Women, Youth)
• Knowledge of best-practice for Specialist Homelessness funded Services

Even Better:

• Previous experience working in Crisis
• Identify and Respond to Risk of Harm training
• Cultural Awareness training
• Mental Health First Aid
