About the role

Reporting directly to the Area Manager, you will be responsible for leading and mentoring a team of Case Managers to help deliver Uniting's innovative model of Home and Community Care across the Central Coast and Hunter Region.

Focused on improving case management practice, not only will you be a key part of forming the local leadership team, there will be the opportunity to input to the wider operations and collaborate with peers across the state.

As a leader you will need to ensure we deliver quality services, achieve packages targets and follow case management best practice while working in partnership with clients to achieve positive outcomes reflecting meaning and purpose for the client.

Playing a vital role within our Home and Community Care business, this role offers you the chance to build upon your own knowledge and could lead to a career in either senior operational leadership or into practice design and implementation with the wider Uniting organisation.

Skills and experience:

  • Tertiary qualifications in Case Management, Aged Care, Allied Health or Community Services
  • Demonstrated supervisory and team leadership experience
  • Well-developed understanding and delivery of case management practice
  • Able to deliver change and work across an organisation to deliver outcomes
