About the role

You’ll be the central point of contact for your clients, supporting them to access the right services and get the most out of their funding, while staying connected to their community.

What you’ll be doing

  • Manage a caseload of clients and lead person-centred care planning
  • Coordinate services aligned to individual needs, goals and budgets
  • Build strong relationships with clients, families and support networks
  • Monitor progress and adjust care plans as needs evolve
  • Identify and escalate risks, including complex or vulnerable situations
  • Work closely with internal teams and external providers to deliver seamless care
  • Support clients to stay independent and connected to their community

What you’ll bring

  • Experience in aged care, community services or case management
  • Strong relationship-building and communication skills
  • Ability to manage competing priorities in a dynamic environment
  • Understanding of person-centred care principles
  • Confidence using systems and maintaining accurate records
  • Experience supporting clients with budgets or funding (desirable)

Essentials:

  • Certificate IV in Aged Care, Community Services or similar
  • Current First Aid Certificate
  • Unrestricted Australian Driver’s Licence
