Provide a flexible and assertive case management response to clients with multiple and complex needs.

  • Ensure that clients with multiple and complex needs within the North West region receive an appropriate service response to assist them to transition out of the homeless system.
  • To work collaboratively within Youth Services Integrated Model, having access to all programs and providing case management support. With acknowledgment of the valuable and dynamic Integrated service delivery that support sustainable outcomes. We are committed to actively working alongside young people and their identified support systems.

Conduct comprehensive assessments of a client's needs and circumstances both at the point of intake and ongoing through support period.

  • Provide intensive case management support to a caseload of young people who are experiencing high and complex needs
  • Duty rosters and procedures are adhered to and followed to ensure ongoing
    response to those requiring crisis support and assessment for service referral
  • Effective relationships are established with clients that encourage self-determination and participation
  • Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
  • Clients are assisted to access support services and establish community supports, and where needed are assisted through advocacy with service providers
  • Progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals
  • Service gaps are identified and recommendations for service delivery improvements are made to the Youth Services Coordinator
  • A strong commitment to working with in a trauma and psychologically informed environment.
  • Work alongside the youth services team at the Sunshine youth hub to provide an assertive response to young people presenting in crisis
  • Participate in the YAC providing secondary consult support to services present
  • Facilitate monthly seminars aimed at developing the capacity of staff within the North west region

YS case manager will operate a case load of up to 12 clients. In achieving the Mission of The Salvation Army, Journey to Independence arranges service delivery into nine central components of care, providing integrated pathways that support young people's physical, emotional, social, spiritual and psychological development. Across all components of care, staff are committed to offering support to young people in achieving outcomes in all six domain areas:

  3. Social Inclusion and Participation
  4. Health and Well-being
  5. Life and Living skills
  6. Relationships

The operational hours are 9am-5pm and YS provides assistance to approximately 450 young people and families per annum.
