The Independent Community Living Program is an outreach service, aimed at supporting individuals who are living with complex mental health issues, to access stable long-term housing, integrate within the community and achieve independent living.
In this role, you will work within a recovery model, facilitating opportunities for social inclusion and supporting individuals to achieve their goals and live independently. Your work begins facilitating the intake process with individuals, identifying their needs and carrying out risk assessment. Partnering with individuals in the development of case plans, you will support individuals to achieve their goals of recovery and to accomplish their aspirations. You will also assist in identifying and securing suitable long-term housing. As you would expect, crisis prevention and intervention are key parts of what you will do. Your case load will be between 10 to 15 people with differing complexity.
You are a compassionate and understanding communicator, with experience in delivering an outreach service. Your experience also extends to advocacy for individuals and working in partnership with individuals with complex mental health. You build rapport and trust with people in challenging circumstances and confidently work with individuals from diverse backgrounds. Your track record demonstrates your involvement in complex situations. Your tertiary qualification is at a minimum of a diploma in social welfare or similar.
The position is permanent full time, 38 hours weekly.
