The Insecure Housing Case Manager links people over the age of 25 years, living in low cost accommodation such as rooming houses, motels, pension level SRS's, social housing settings and private hotels, to relevant services within the City of Stonnington and Port Phillip. The clients are usually seen in an outreach setting in their own accommodation.

We are currently looking for experienced Case Managers, with experience in Homelessness or Housing, based across Melbourne's Southern Inner Suburbs.

You will be responsible for delivering a broad range of services to people who are homeless or at risk of homelessness and in crisis, or transitioning to short-term/long-term housing.

You will:

• Provide an immediate respond to referrals into the program
• Hold an average case load of 12 clients
• Help clients navigate government services
• Provide comprehensive face to face assessments
• Provide case management to clients who are homeless or at risk or homelessness
• Develop and implement collaborative, holistic, client-focused case plans
• Establish and maintain effective relationships with key support services
• Undertake community capacity building activities to improve response to vulnerable people

You will have:

• A tertiary qualification in Social Work, or a relevant Health Sciences degree
• Competency in Microsoft programs, SHIP, CRIS or other Case Management software - highly desirable
• Assessment skills, mental health and housing experience is preferred
• Indigenous links or understanding would be regarded highly
• Demonstrated knowledge and ability to respond to people with needs, via responsive outreach models and assessments
• Demonstrated understanding of the homelessness service system
• A valid and current Victorian driver's licence

You will also have or be willing to undergo a Working with Children Check and a Police Check.
