An exciting opportunity is available for an Aboriginal and Torres Strait Islander Health Worker to join a multidisciplinary healthcare team supporting young people within a youth justice custodial environment.

This role focuses on delivering culturally safe, person-centred care and supporting Aboriginal and Torres Strait Islander young people from reception through to release into the community.

This is an identified position and only Aboriginal and/or Torres Strait Islander people are eligible to apply under Section 12 of the Equal Opportunity Act 2010 (Vic), which permits special measures to promote substantive equality for Aboriginal and Torres Strait Islander peoples.

The Role:

You will work closely with young people within a custodial setting to support their physical health, mental health, wellbeing, and connection to services both during custody and as they transition back into the community.

The role involves building strong therapeutic relationships, reducing barriers to accessing healthcare, advocating for young people, and working collaboratively with clinical teams, families, and community organisations to ensure holistic care outcomes.

Key Responsibilities:

  • Engage with Aboriginal and Torres Strait Islander young people in a culturally safe and respectful manner
  • Support young people with healthcare access, advocacy, and care coordination
  • Assist with health promotion, prevention programs, and wellbeing initiatives
  • Develop and maintain goal-directed care plans
  • Monitor and document client progress and health needs
  • Work collaboratively with multidisciplinary healthcare teams and external stakeholders
  • Build and maintain relationships with Aboriginal community organisations and support networks
  • Support young people transitioning back into the community
  • Maintain accurate and compliant documentation and case notes

About You:

  • Aboriginal and/or Torres Strait Islander person (identified role)
  • Experience working within community health, youth services, justice, mental health, AOD, or social support environments
  • Strong understanding of culturally safe practice and trauma-informed care
  • Ability to build rapport and engage with vulnerable young people
  • Strong communication, advocacy, and stakeholder engagement skills
  • Passionate about improving health outcomes for Aboriginal and Torres Strait Islander communities

The Environment:

This role is based within a youth justice custodial setting supporting young people aged 10-21 years. You will be part of a nurse-led health service delivering primary health, allied health, dental, and mental health supports in a collaborative and integrated environment.
