You will lead, coach and support a team of Aged Care Case Managers to deliver high-quality, person-centred services that create meaningful outcomes for older Australians accessing in home care through both the Support at Home Program and Commonwealth Home Support Program (CHSP).
You will drive a positive team culture focused on accountability, collaboration and continuous improvement, while ensuring service delivery aligns with aged care standards, organisational goals and client expectations.
Using your leadership and clinical expertise, you will provide guidance on complex cases, support staff development, and foster a high-performing team environment where people feel empowered to do their best work.
