About the role

As our community-based Physiotherapist, you’ll play a critical role in enhancing the health, mobility and wellbeing of older people across Western Sydney, supporting them to maintain independence and live safely and confidently at home.

You’ll create meaningful experiences for older people through warm, engaging and person-centred care. Working within a strong clinical governance framework, you’ll take a proactive and thorough approach to assessment, intervention and ongoing client support. You’ll identify risks early, develop tailored treatment plans and collaborate with multidisciplinary teams to deliver holistic care that promotes safety, dignity and quality of life.

Duties

• Provide in-home physiotherapy assessments and treatments tailored to each client's goals, ensuring timely documentation and care aligned with wellness and enablement principles.
• Identify risks and changing client needs early, ensuring timely interventions and appropriate safeguarding responses.
• Develop holistic, evidence-based care plans for clients with complex needs, collaborate with multidisciplinary teams, and advocate for client autonomy and quality of life.
• Adhere to the Aged Care Act 2024 and Quality Standards, contribute to continuous service evaluation, and uphold high clinical governance standards to maintain top-tier service outcomes.
• Travel to suburbs across the service area including: Penrith, Blacktown, The Hills District, Parramatta, Liverpool and Campbelltown.

Skills and Requirements

• Tertiary qualification in Physiotherapy with current AHPRA registration is essential.
• 12+ months experience providing physiotherapy services
• Valid First Aid & CPR certificate
• Current drivers licence with a willingness to travel.
• Strong understanding of person-centred care and contemporary aged care clinical practices.
• Ability to build genuine rapport with clients and deliver compassionate, engaging and high-quality client service.
• Excellent time management skills, professional accountability and commitment to service quality and continuous improvement.
