About the role

As ourCare Partner, you’ll provide phone-based clinical case management and support to older people across Greater Sydney, helping them maintain independence, dignity, and quality of life while living at home.

The role has a strong clinical focus, supporting clients with complex and evolving health needs, including those requiring higher levels of care and end-of-life support. You will use your clinical expertise to assess needs, develop tailored care plans, and coordinate services that align with each client’s goals, preferences, and stage of life.

Working alongside Community Care Workers and a broader multidisciplinary network, you’ll ensure care is coordinated, responsive, and aligned with a consumer-directed care approach.

Key Responsibilities

• Provide phone-based case management and support to clients under the Commonwealth Home Support Programme (CHSP) and Support at Home (SAH), including completing assessments and developing personalised care plans.
• Use a relationship-focused approach to identify each person’s goals and abilities.
• Build and maintain strong networks with agencies, health professionals, and community services, including ACATs, GPs, social workers, and culturally specific organisations.
• Provide person-centred case management in collaboration with clients, families, and carers.

Skills and Experience

• Demonstrated case management experience across the Support at Home & Commonwealth Home Support Program.
• Ability to work in a person-centred, holistic way to achieve positive outcomes for clients.
• Strong interpersonal and customer service skills, with the ability to communicate effectively with people from diverse backgrounds.
• A collaborative team player who can balance competing priorities while maintaining self-care.
• Excellent organisational skills and confidence to represent the service at community events.
• Current driver’s licence is essential.
• A relevant qualification in case management, social work or ageing services is desirable.
