As ourCare Partner, you’ll provide phone-based clinical case management and support to older people across Greater Sydney, helping them maintain independence, dignity, and quality of life while living at home.
The role has a strong clinical focus, supporting clients with complex and evolving health needs, including those requiring higher levels of care and end-of-life support. You will use your clinical expertise to assess needs, develop tailored care plans, and coordinate services that align with each client’s goals, preferences, and stage of life.
Working alongside Community Care Workers and a broader multidisciplinary network, you’ll ensure care is coordinated, responsive, and aligned with a consumer-directed care approach.
