As a Case Manager in our Street to Home program, you will deliver person-centred case management and assertive outreach to individuals experiencing homelessness and complex disadvantage. Working collaboratively with companions, you will support them to identify their goals, address barriers, and access the services and resources needed to achieve greater independence and long-term stability.
You will play a key role in connecting companions with housing, healthcare, legal assistance, income support, and other essential community services. In addition, you will support the delivery of our six-unit transitional accommodation program, helping to foster a safe, respectful, and supportive environment for companions as they work towards sustainable housing outcomes.
This is a fixed-term, part-time opportunity for 10 months, based in Yarralumla, ACT, working 9 days per fortnight on weekdays.
We are looking for a dedicated and adaptable professional who thrives in a fast-paced environment. In this role, you will manage a diverse and complex caseload across both field and site-based settings, demonstrating initiative, sound judgement, and the ability to work independently while achieving positive outcomes for companions.
You hold qualifications in Social Work, Community Services, or a related field, with experience in case management, preferably within homelessness services or a similar sector. You have excellent communication and interpersonal skills, and the ability to manage competing priorities in a dynamic environment.
To be successful in this role, you must hold a current driver’s licence and will be required to complete a National Police Check and hold, or obtain, a Working with Vulnerable People Card (WWVP). A First Aid Certificate or willingness to obtain within three months of commencement is required.
