About the Role
We are seeking a passionate Dietitian to join our multidisciplinary Chronic Care Team. This role delivers both clinical dietetic services and community nutrition programs across the Sunrise region.
You will:
- Provide individual dietetic assessments and counselling (clinic and home visits)
- Deliver group education sessions and health promotion activities
- Support management of chronic conditions including diabetes, cardiovascular disease, renal disease, and malnutrition
- Work collaboratively with Aboriginal Health Practitioners, Nurses, and Care Coordinators
- Participate in outreach trips to remote communities
- Develop culturally appropriate nutrition resources and programs
- Contribute to food security initiatives (e.g. community programs, local partnerships)
- Maintain clinical records, reporting, and data systems
- Support continuous quality improvement (CQI) and service integration

Skills & Experience
- Degree in Human Nutrition and Dietetics
- Eligibility for Accredited Practising Dietitian (APD) status
- Ability to obtain a Medicare Provider Number
- Experience working as a practising Dietitian
- Strong understanding of primary health care and chronic disease management
- Experience working in a multidisciplinary team
- Knowledge of Aboriginal health and culturally safe practice (highly regarded)
- Ability to work independently and manage competing priorities
- Current manual driver’s licence

- National Criminal History Check (non-adverse)
- Current OCHRE Clearance
- Current First Aid Certificate
- Up-to-date vaccinations
- Full working rights in Australia
- Willingness to travel and work in remote communities
