The Primary Integrated Carers Supports (PICS) program provides coordinated clinical care and psychosocial support to people with severe and persistent mental illness living in the Central and Eastern Sydney region who can be supported in primary care. The program is delivered within a stepped care model to ensure strong linkages to general practice and bulk-billed psychiatric services.
PICS is delivered by credentialed mental health nurses and peer workers, who work collaboratively, to support people to transition to GP care.
The Peer Worker will work as a team member with the PICS Peer Coordinator and PICS Credentialed Mental Health Nurses to provide support to meet each person's needs and wishes to enable them to live well and achieve their recovery goals. Key responsibilities will include:
