The Program

The Primary Integrated Carers Supports (PICS) program provides coordinated clinical care and psychosocial support to people with severe and persistent mental illness living in the Central and Eastern Sydney region who can be supported in primary care. The program is delivered within a stepped care model to ensure strong linkages to general practice and bulk-billed psychiatric services.

PICS is delivered by credentialed mental health nurses and peer workers, who work collaboratively, to support people to transition to GP care.

About the Role

The Peer Worker will work as a team member with the PICS Peer Coordinator and PICS Credentialed Mental Health Nurses to provide support to meet each person's needs and wishes to enable them to live well and achieve their recovery goals. Key responsibilities will include:

  • Build one to one supportive relationships based on peer support values that focus on the use of lived experience to support the people we work with in regards to the persons needs and recovery.
  • Work with the nurses, in the development of the individual care plans with clarity of identified roles and tasks.
  • Provide ongoing support to the person within the context of the care plan and goal setting.
  • Provide the person with education and strategies for the development of self-management and wellness skills using personal lived experience.
  • Build on knowledge of current services in the area to link clients to relevant services. Be able to refer to other services when needed. Share this information with the whole team to build up the knowledge of available services.
  • Assist the person to navigate the system and provide support to the client to access new and necessary services. To provide advocacy with Centerlink, FACS Housing, Psychiatrists, Psychologists, GP's and other allied health professionals and other services as required.

About You

To succeed in this role, you will have:

  • Certificate IV in Mental health Peer Work or completion within 12 months of commencement in the role.
  • Lived experience of mental illness and the ability to use the experience to support others
  • Knowledge of the NSW mental health system and local services
  • An understanding of the recovery- based approach to assisting people with severe and persistent mental illness
  • The ability to communicate effectively in all interactions with all stakeholders
