From 1 January 2026, Sunbury and Cobaw Community Health and Nexus Primary Health have merged to form a new, unified not-for-profit community health organisation, Omnia Community Health. This decision followed an extensive exploration process that confirmed the merger will improve access to safe, high-quality, and sustainable health and community services across the Hume, Macedon Ranges, Mitchell, Murrindindi and Strathbogie local government areas. The new organisation will bring together the deep community connections, shared values and specialist expertise of both entities. Services will span general practice, allied health, disability support, mental health, early childhood, family services, aged care and community wellbeing, delivered from sites in Broadford, Kinglake, Kyneton, Romsey, Seymour, Sunbury and Wallan, with additional outreach into surrounding communities.

About the Role

At Omnia Community Health, we believe that healing happens in safe, connected and inclusive spaces. As a Therapeutic Counsellor in our CATCH (Children, Adults, Therapy, Counselling and Healing) team, you’ll play a key role in supporting people to make sense of their experiences, build resilience and move toward the life they want.

This is more than a counselling role—it’s an opportunity to work alongside a multidisciplinary team that values collaboration, reflection and genuine care for both clients and staff.

Key Responsibilities

  • Provide trauma-informed, evidence-based counselling to children, young people and adults, applying established therapeutic frameworks within a defined scope of practice
  • Undertake structured psychosocial assessments and develop, implement and review client-led care plans to support therapeutic goals and outcomes
  • Deliver individual therapeutic interventions and, where appropriate, contribute to group programs across centre-based and outreach settings
  • Exercise sound professional judgement in managing client presentations, seeking guidance and escalation for complex or high-risk situations in line with organisational protocols
  • Liaise with relevant internal and external services to facilitate referrals, continuity of care and holistic support for clients
  • Maintain accurate, timely and compliant case notes and client records in accordance with organisational, legal and professional standards
  • Comply with organisational policies, procedures and safeguarding responsibilities, including privacy, confidentiality and child safety requirements

Qualifications and Compliance

  • Bachelor’s in social work or equivalent therapeutic qualifications and work-related experience in infant, child and adolescent related disciplines
  • Most hold current National Criminal History Check, NDIS Worker Screening check and valid Working with Children Check
  • Must hold current Victorian Driver’s Licence
