Lead meaningful Aged Care outcomes as a Senior Client Coordinator with Rise covering Perth’s eastern suburbs.

Rise is seeking a Senior Client Coordinator to support clients across Support at Home and Fee for Service programs, covering High Wycombe, Kalamunda, Lesmurdie, Midland and surrounds.

This is a Monday–Friday, 8:30am to 4:36pm role with a hybrid setup (community travel, work from home + Rise office locations). 

The role in a nutshell

• Managing a portfolio of up to 60 clients and ~$2M in funding, ensuring services are tailored, well-coordinated, and truly client-directed
• Operating with a high level of autonomy, making decisions within program guidelines
• Acting as the primary contact and advocate for clients, building strong, trusted relationships
• Developing and reviewing care plans, services, and budgets aligned to client goals and funding
• Monitoring outcomes, managing risks, and adjusting supports as needs change
• Working with providers and internal teams to ensure services are delivered effectively
• Overseeing budget performance, reporting, and compliance, while identifying improvements in service delivery

What you’ll bring

• Experience in home care, with a strong understanding of the Support at Home program and relevant legislation (or comparable sector experience)
• Demonstrated ability to assess client needs and coordinate services that deliver meaningful, person-centred outcomes
• Certificate IV in Ageing Support, Community Services, or equivalent practical experience
• Strong client focus with sound judgement and decision-making capability
• Confidence managing budgets, with the ability to work autonomously
• Clear communication skills and strong organisational ability
• Solid administrative and IT skills 

Bonus: Clinical background or relevant tertiary qualifications
