A varied, community-based role providing client support and service continuity across Aged Care.

The need-to-know…

The Relief Support Worker plays an important role at Rise, ensuring continuity of care across our Aged Care services by providing flexible support where it is needed covering leave and vacant services in the community. The position includes delivering in-home support to clients, assisting with activities of daily living, promoting independence and social connection, and providing general domestic assistance where required. In addition to direct client support, you will assist with onboarding new clients, providing initial services, and supporting the Scheduling Team with client communications when required. 

This is a full-time position initially offered on a 12-month contract, working Monday to Friday with shifts between 7:00am and 6:00pm (76 hours per fortnight). The role requires travel throughout the South Metro, including Byford, Victoria Park and surrounding suburbs, with daily schedules and travel times varying based on service needs. 

Important note: This is a community-based Aged Care position that involves travelling between multiple client homes throughout the day. Applicants must be willing and able to undertake daily travel as an essential requirement of the role.

The need-to-have…

Here at Rise, we nurture potential, but we do need you to have a few tricks up your sleeve including the following: 

  • Certificate III (Community Services) and/or related qualification with experience in aged care support work 
  • Demonstrated ability to work well with people who are aged
  • Ability to bend, kneel and lift correctly to complete duties safely
  • Driving licence and reliable vehicle to use for work purposes
  • First aid certificates (HLTAID009 & HLTAID010) or willingness to obtain 

Formalities (The serious stuff!) 

Appointments to this position will be subject to a national police clearance. Costs for new checks will be covered and/or reimbursed by Rise.
