In this role, you will support clients within the Independent Living Program (ILP) across the Swan and Northam regions. Using a recovery-focused approach, you will assist up to 78 clients living with severe and persistent mental health conditions to maintain stable housing and sustain their tenancies.
This is a varied role combining outreach and office-based work, with most time spent in clients' homes and the community. A hybrid work arrangement may be available once you are established in the role.
The ILP program is funded by the Mental Health Commission, with support tailored to individual client needs, ranging from low to high intensity. You will work closely with the Rise Property Team, clinical services, and broader support networks to provide coordinated, wrap-around support as clients transition from homelessness or housing instability into social housing.
It's a highly rewarding position, where you will provide vital tenancy support to vulnerable clients, alongside case management, advocacy, and referrals to appropriate services.

Appointments to this position require a NDIS worker screening check and working with children check. Costs for new checks will be covered and/or reimbursed by Rise.
Additionally, candidates must hold a current driver's licence with own vehicle to use for work purposes (paid KM's for use of personal car).
