Every journey home is different, but no one should have to navigate it alone. Join our Hospital to Home team in Caboolture as a Recovery Support Worker on a 12-month fixed-term, full-time contract, where you'll play a vital role in supporting people as they transition from acute mental health care back into their community. Through practical, person-centred psychosocial support, you'll empower individuals to build confidence, strengthen everyday living skills, reconnect with their community and work towards the goals that matter most to them.Key duties include:

  • Supporting clients through hospital discharge planning and transition to the community
  • Visiting clients in hospital and providing follow-up recovery support at home
  • Working alongside clients to achieve goals identified in their Personal Recovery Plans
  • Assisting with day-to-day tasks, skill development and capacity building
  • Facilitating community connections and social inclusion
  • Collaborating with internal and external stakeholders to achieve the best possible outcomes
  • Managing your own client caseload with ongoing supervision and team support

Make a meaningful impact by empowering people to build independence and live well in their community.You will have:

  • Diploma in a human service-related field and/or demonstrated previous experience working with people with mental illness.
  • Ability to work autonomously and professionally within RFQ's service philosophy and values.
  • Demonstrated ability to effectively manage time, own case load and KPI's including outcomes measures and safety planning.
  • Excellent communication and interpersonal skills with the ability to effectively engage with clients, colleagues, stakeholders, and service providers.
  • A strong sense of professional boundaries and integrity.
  • Willingness and ability to undertake vaccination of preventable diseases that complies with the Occupational Assessment, Screening & Vaccination against Specified Infectious Diseases.
