About the role

A tremendous role that will see you reporting to the Chief Operating Officer, were you will take ownership of the planning, delivery, quality and performance of Home Support services across the Commonwealth Home Support Program (CHSP) and Support at Home programs. You will be considered the operational lead for these services, ensuring they are consumer-centred, compliant, financially sustainable and aligned to best practice.

Duties and Responsibilities

  • Lead the end-to-end operations of CHSP and Support at Home programs, ensuring service targets, quality standards and budget expectations are achieved
  • Develop and implement operational plans and growth strategies, including readiness for ongoing sector reforms
  • Partner closely with the Clinical Care Manager to integrate clinical and non-clinical service delivery, ensuring clear escalation pathways and scope of practice
  • Oversee intake, service delivery, rostering and ongoing service adjustments in line with consumer needs and care plans
  • Manage program budgets, financial performance and reporting, including funding claims and acquittals
  • Provide leadership, coaching and support to coordinators, support staff and volunteers, fostering a positive and accountable team culture
  • Support recruitment, onboarding and workforce development in partnership with HR
  • Build and maintain strong relationships with consumers, families, referrers and community stakeholders

Skills, Experience and Qualities

  • Demonstrated experience managing community aged care or home care programs such as CHSP, HCP or similar, including leading multidisciplinary teams
  • Strong working knowledge of Aged Care Quality Standards, funding models and the evolving Support at Home framework
  • Proven ability to manage budgets, service performance and operational outcomes using data and insights
  • A strong understanding of person-centred, community-based service delivery
  • You are innovative in your thinking, you ask why and demonstrate curiosity. You have a growth mindset and a focus on continuous improvement
  • Highly developed stakeholder engagement, communication, and problem-solving skills
  • A proactive, solution-focused approach with the ability to lead through change
  • Tertiary qualifications in Health, Community Services, Business or a related field will be highly regarded
