Plan Navigators started in mid-2018 as a division of Inclusion WA. We set up the service to assist people with disability to take an increased level of control of their funding and their services. Plan Navigators has two objectives:

  • To make self-direction easier, so people can manage their own NDIS funding with confidence and creativity.
  • To assist people with disability to attract and retain the best possible staff to help them achieve what they want to achieve in life.

We are seeking a motivated, passionate, creative Service Coordinator who lives in and understands regional WA to support the growth of our regional work in the Great Southern Region. The role is to work alongside people living with disability and/or mental illness and their families to support them to take an increased level of control of their NDIS funding, their services and their supports. This role requires a degree of flexible and creative thinking to support people living in smaller regional towns to build their supports and services around them.

This position is a part time role with fixed hours of 30.4hrs per week and is fixed term for a 12-month period with a view to extend. Reporting to the General Manager, your role will include, but is not limited to:

  • Assist people to take an increased level of control in all aspects of their lives
  • Assist people to understand how they can use their NDIS funding, how to manage their funding and how to implement their plan.
  • Supporting people through their NDIS plan reviews
  • Assist people (using innovative approaches) who live in smaller regional towns to employ their own staff and engage services that will meet their needs and situation
  • Collaborating with key people in the person’s life, including family members, employers or other support services to achieve the best outcomes for the person
  • Investigate local community opportunities for people you support to participate in and help implement strategies to reduce social isolation
  • Developing strong working relationships with services in regional areas and building new connections
  • Working closely and collaboratively with providers in regional areas
  • Working alongside people who may be living through complex circumstances
  • Travelling to meet with people and their families, supports and services throughout the Great Southern Region
  • Working from home and being part of a ‘virtual’ team
  • Write reports and collect evidence for participant plan reviews
  • Developing workshops that can be used to upskill NDIS participants to take more control of their funded supports and services

Required

  • Must live in the Great Southern Region
  • The ability to maintain high level of personal and professional boundaries when working with people who live in the same regional community
  • Strong understanding of the National Disability Insurance Scheme (NDIS) and a passion for supporting people to get the supports they need
  • Previous work or lived experience with people who have an NDIS plan
  • A strong commitment to social justice issues and social inclusion principles
  • Strong customer service skills and the ability to build rapport quickly
  • The ability to write reports required for NDIS
  • Being able to work autonomously and as part of a team
  • Being able to work from a home office arrangement
  • Being able to travel for work due to service people living in the Great Southern Region
  • Strong administrative, time management and problem solving skills
  • Tech saVvy
  • Being able to work under pressure and meet deadlines
  • The ability to be a strong and effective communicator with a variety of stakeholders
  • The ability to interpret and communicate financial information
  • A willingness to learn new skills and develop knowledge gaps

Desirable

  • Lived experience of disability, mental health illness, Aboriginal and Torres Strait Islander people, Culturally and Linguistically Diverse people, and LGBTQIA people
  • Approachable nature and a great sense of humour
  • Facilitating workshops and developing the content
  • Good understanding of Shared and Self-Management principles
  • Previous work experience as a Support Coordinator or Plan Manager
  • An in-depth understanding of the NDIS price guide
  • Ability to use Excel spreadsheets
  • Ability to use Office 365 including Microsoft Teams

Selection process:
