The Role

Because of our continuous growth, we are seeking a dedicated, full-time Community Worker to join our team. In this role, you will be at the heart of what we do, assisting clients with community inclusion, meaningful participation, and access to employment.

While our head office is located in Moe, our support workers operate across Victoria. You will be assigned to participants at various locations, providing hands-on support directly in the clients' homes or within Supported Independent Living (SIL) houses. This is a highly mobile role, requiring you to commute and drive between different participant locations to deliver consistent care.

Key responsibilities include:

  • Assessing client needs, strengths, circumstances, and specific support requirements to help them live more independently
  • Assisting clients with community inclusion, social participation, and accessing employment or training opportunities
  • Providing direct, compassionate support to participants at their own homes or within SIL houses
  • Developing and implementing individual support plans that promote well-being, life skills, and inclusion
  • Liaising closely with families, carers, service providers, and community organisations to coordinate the best possible care
  • Monitoring client progress, maintaining accurate case notes, and preparing detailed service reports
  • Helping clients access vital health, housing, education, training, employment and community services
  • Providing guidance, advocacy and referrals to appropriate welfare and community services to ensure comprehensive care
  • Commuting safely and reliably between various participant locations across the state

Ideal Candidate:

  • Possesses a valid driver’s licence with no suspensions and has access to a reliable vehicle
  • Holds an NDIS Worker Screening Check and a Working with Children Check (WWCC)
  • Holds a Diploma in Community Services or a related field
  • Has a current First Aid and CPR certificate
  • Can provide a current National Police Check
  • Demonstrates strong interpersonal skills for engaging effectively with clients, families, and stakeholders
  • Has the ability to maintain detailed case notes and prepare accurate service reports
