About the role

Reporting to the Director, Strategy Implementation and Engagement, the Manager, Population Health and Evaluation will lead high-quality monitoring, evaluation, and population health initiatives across NWMPHN. The role focuses on translating data and insights into practical recommendations that guide commissioning, and organisational decision-making.

You will work closely with the Insight, Performance and Digital Services (IPDS) team and senior leaders to ensure population health intelligence and evaluation findings inform planning. You will also mentor and support a small, high-performing team to deliver measurable impact across commissioned programs.

This collaborative, hands-on role suits a data-driven leader with strong population health and evaluation expertise, and a passion for applying insights to drive continuous improvement and better community health outcomes.

What will my typical week look like?

• Act as a trusted advisor and technical liaison to business stakeholders, building strong relationships to promote solution adoption, continuous improvement, and user-centred design
• Translate complex business challenges into high-quality business and technical requirements and supporting documentation to enable robust technical solutions.
• Identify and lead opportunities for process improvement and automation, applying systems and design thinking to streamline workflows, enhance system integration, and ensure best practice is consistent across the business
• Collaborate with architecture, software development, and data analytics teams to define scalable and integrated solutions to meet evolving business and technical needs.
• Coordinate with the service delivery team on timelines and scope to ensure key milestones are achieved to ensure quality of analysis and documentation

About you

• Tertiary qualifications in public health, epidemiology, evaluation, health economics, or other related areas.
• Extensive experience in monitoring, evaluation, and population health planning, ideally in the health or community services sector.
• Strong skills in data analysis, interpretation, and translating insights into practical recommendations.
• Demonstrated experience with Health Needs Assessments and using population health intelligence to inform commissioning.
• Excellent written and verbal communication skills, with the ability to present complex information clearly to diverse audiences.

Desirable

• Experience working within or alongside a Primary Health Network or similar organisation.
• Experience in providing consulting services, including the ability to engage with stakeholders, analyse complex issues, and deliver practical, evidence-based recommendations.
