It is an expectation that the clinician will work with GP's and/or psychiatrists and care will be delivered in line with a GP Mental Health Treatment Plan or equivalent, developed by the GP or psychiatrist. Offer the right frequency and volume of services to meet individuals needs and are consistent.
The Clinical Care Coordinator will work and promote the use of a single multiagency health care plan to deliver focused interventions for individuals with severe and complex mental health needs in a primary care setting.
The position will be responsible for providing individual/group clinical therapeutic services to clients within Port Lincoln and surrounding areas. Provide evidence-based therapies for people with severe mental health needs. The priority of Clinical Care & Coordination is planning and improving physical health and wellbeing for people living with mental illness.
